Patient Billing Information

It is the patient’s responsibility to know and understand their insurance benefits, including authorization needs, prior to first appointment.

Common Billing Questions

Statements are emailed on the 15th of every month to the primary email address attached to the chart. Patients can also access the statement on their patient portal at any time.

Payment in full is due within 30 days of statement date.

Payment is accepted either in person at any of our locations, over the phone, via mail or through our patient portal. We accept Visa, Mastercard, Discover, cash or check.

Yes! We work with our patients one-on-one to set up payment plans based on their treatment needs and financial struggle. Payment plans have a minimum of $100 monthly payment and payments can be set up to run automatically on a credit card. We charge no late fees, and no interest will accrue on open balances. Please call the office to start the payment plan process.

All receipts can be found on the patient portal under the billing section. Each receipt will have an informational icon next to it which provides details on how the payment was distributed.

In accordance with Federal HIPAA Guidelines and 42 CFR, to protect the privacy of our patients, a signed release of information is required to share specific information about our patients aged 16 years and older, regardless of relationship.
